Iowa Senior Health Program
The Iowa Senior Health Program (ISHP) is available to seniors ages 55 and older. The program is designed to promote and maintain the health of older adults. An ISHP screening clinic provides these services:
- Detailed health history
- Nursing physical assessment
- Selected screening test (e.g., visual, hearing, blood / urine, blood pressure)
The screening takes approximately two hours and is available by appointment. The ISHP program is provided through a grant from the Iowa Department of Public Health; there is no fee, but donations are accepted. Please call 712-737-2971 for an appointment.
Homemaker Services
Homemaker home health aide services are provided to clients in their homes to enhance their capacity to attain or maintain their independence. They are provided when the homemaker experiences limitations, stress or crisis. Homemaker services will be provided to clients in homes within a 30-mile radius of the Sioux Center Hospital. All clients are evaluated for appropriate care on an individual basis.
Chears Home Care
Home health care services are provided to patients and their families in their home settings. Care is provided to patients of all ages who are disabled, chronically ill or recovering from an illness or surgery. Services are provided to patients residing in a 30-mile radius of the Chears office.
Chears is a licensed and certified provider of home health care. Many services are paid for by Medicare, Medicaid or other insurance carriers. An initial evaluation visit is made free of charge. Chears will provide services regardless of a family's ability to pay.
Chears Hospice
The Chears hospice offers a coordinated program of services to terminally ill patients and their families. Care is provided in a patient's home as well as in the hospital and nursing home settings. Physical, emotional, social and spiritual care is provided by a hospice team, which includes a physician, nurse, home health aide, social worker, pastor, counselor and volunteers.
Hospice care is supportive care for the patient and family, focusing on managing the pain and symptoms of the disease as well as providing spiritual and emotional care. For information, call 712-722-1271 or 800-722-1922.
